Use pd.cut instead of pd.qcut for step function in Ch07 (#81)

Equal-width bins match the cutpoints of Figure 7.2. The pd.qcut fit is
kept further down, where it is compared with the degree-0 B-spline fit
whose knots are at the age quartiles. Supersedes #42.
